Authors Membership Summary Background: Intraoperative collangiography during laparoscopic cholecystectomy reveals the anatomy of the bile tree and the stones contained therein. The use of intraoperative collangiography can be routine for all laparoscopic cholecystectomy. An alternative approach is a selective policy, implementing intraoperative collangiography only for those cases where collodocolytosis is suspected for clinical reasons, or those for which anatomy seems unclear in operation. Literature on both approaches is examined to define their respective merits. Methods: The relevant English articles were identified in the Medline database and revised. Results: The revised literature consisted of retrospective analysis. In general, the incidence of unsuspecting stones was 4%, but only 15% of them would continue to cause clinical problems. The incidence of the complete transsection of the common bile duct was rare for both routine and selective intraoperative choreangiography policies, and does not differ between them. Less lesion rates in the bile ducts do not differ between groups, but it is more likely to be recognized in the routine group than in the selective (P = 0.01). Conclusions: Routine intraoperative choreangiography produces very little useful clinical information about what is obtained with selective policies. A large number of unnecessary intraoperative choreangiography is performed under the routine of choreangiography intraoperative policy, and therefore a selective policy is defended.
